Retirement forms must be submitted through Retire Now, ERF's retirement portal. You can access the forms below or through the portal but they will only be accepted through Retire Now. Contact us at retirement_fund@dallaserf.org or 214-580-7700 to set up your account.

ERF Beneficiary and Tier B member forms are available below for your convenience. To submit a form, locate the form you need below and click on the form title. Your form will open in a new window. The required fields will be highlighted for you to type in directly.

There are four ERF forms that need to be notarized; Retirement Application, Refund Application, Beneficiary Application and the Direct Deposit Authorization.
