Most ERF forms are available online for your convenience. To submit a form, locate the form you need and click on the form title. Your form will open in a new window. The required fields will be highlighted for you to type in directly.
Once you have completed the form, print it out, sign it (unless it is stated on the form that a notary is required) and either mail or hand deliver it to ERF.
There are three ERF forms that need to be notarized; Retirement Application, Refund Application and Beneficiary Application. For your convenience, a notary is available in the ERF office.
